Students can use a template for a student self-evaluation form to gauge their own academic performance and offer input on their development.

This assessment form is meant to help students reflect on their own learning and take ownership of their academic development.

To make your survey more interactive and unique, you can include answer piping. Then, be notified and get an email whenever the form receives a new response.

You can create a Kanban board to keep track of your progress each time you submit this form. You can also create advanced charts to check the evolution of your scores and compare them together throughout the academic year.

You can also share the form with your teachers and add a field for them to give you feedback. Manage their access depending on what you want them to perform on your evaluation form.
